Remuneration: £400 per day
Description: Part Time Locum Dentist required 8 days in December on a Mobile Unit in Ipswich, Felixstowe & Bury St Edmunds
• £400 per day
• To cover Mobile Dental Services with pick up points Ipswich, Felixstowe and Bury St Edmunds
• Providing services to Special Educational Needs Schools, Local Community Centres, vulnerable adults and Children
• Hours 8.30am - 4.30pm with half hour lunch
• Nurse and Mobile Driver on board
• Days 3rd Bury St Edmunds, 4th Ipswich, 5th Ipswich, 11th Ipswich, 12th Felixstowe, 17th Ipswich, 18th Felixstowe & 19th Ipswich December

- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate

- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
