Job Details
This Mixed Dental practice are looking for an urgent Locum Dental Associate to cover :
Monday 08:00 - 14:00 or 08:00 - 20:00
Tuesday 10:00 - 18:00
Start date 1st July
Rates very depending on the hours worked.
£300 day rate for 25 UDAS For the 6 hour shifts (8-2/2-8)
£350 day rate for 30 UDAS for the 7.5-8 hour shifts (9-5/10-6pm)
£400-450 day rate offering between 35-40 UDAS for the Longer shifts (8-6pm/8-8pm)
50% private and 50% lab bills.
£10 for underachieving the target.£12 for overachieving.
Very busy, packed book to take over from.
All candidates must be fully qualified and GDC registered with an active performer number and UK experience.

GDC CERTIFICATE
HEP B IMMUNISATIONS
INDEMNITY CERTIFICATE
CRB/DBS CERTIFICATE
2 FORMS OF ID
2 REFERENCES/REFEREE DETAILS
